The last seven days only
What actually happened this week?
Tick against the last seven days, not against the illness in general and not against what you are dreading.
Nothing you tick leaves this page.
You ticked more than four
Take four
Choose the ones that cost you the most time or the most upset, not the ones that worry you most about the future. The future sheets will still be here.
Which of these would help you?
Pick as many or as few as you like. Nothing here is a test.
Your sheets for this week
Print these, and only these
A wall of paper is worse than eight correct pages. Run this again in a month, because the answer moves.
Pay once. Reprint for as long as you need.
The sets
Three sets. The Person set holds the sheets written for the person living with memory loss to hold and read. The Caregiver set holds the sheets the person helping holds. The Full set is both parts together.
Not sure which stage? Answer a few questions about the last seven days and the site will name the set for you. Choose by what you see this week, not by a number.
Your set
What you get
- One file to print, plus each sheet on its own
- The files stay in your Etsy purchases, so you can download and reprint them again any time
- Paid once. No subscription, nothing renews
The buy buttons open the CareSheetsCo Etsy listing in a new tab. Etsy handles the payment, so we never see or store your card details.
For care communities and home-care agencies
A site license
One license covers every stage and every resident or client you serve. Buy it once; print as many copies as your staff need, for as long as you use them.
Every stage, every resident
A unit holds residents across the whole range. The license includes all fifty-nine sheets, both parts, so staff print what each person needs this week.
Built for shift change
The partner sheets double as handover tools. Sheet 23 passes the day on, Sheet 56 shows who owns each hour, and Sheet 57 holds the decisions nobody should make alone at 3 am.
Training that happens while working
A new aide who reads a step card learns the practice while doing it. The sheets support your required dementia training; they do not replace it.
What the license includes
- Print-ready files for your print shop, on the specified cream matte stock
- Permission to print for every resident or client you serve
- For home-care agencies, permission to print sets for your clients' families
- Sized by community or agency size, paid once, with an invoice and purchase order if you need one
- An optional yearly refresh with new sheets and translations. It never renews without a reminder first
The license is one-time to the set purchased: one license per set, at the resident count at time of purchase. It is not a forever-license to everything released later. New sheets and translations come through the yearly refresh.
An organizing tool, not medical advice. The care plan comes from the clinician.
Addendum
Important use and medical disclaimer
Please read this addendum together with the Memory Care Sheets. If an individual sheet is printed or shared separately, this addendum should remain available to the person using it.
Educational and organizational use only
The Memory Care Sheets are general educational and organizational materials intended to support people living with memory loss and the people who assist them. They are not healthcare services and are not medical, nursing, psychological, nutritional, rehabilitation, speech-language pathology, occupational therapy, physical therapy, or other professional advice. They are not intended to diagnose, treat, cure, prevent, or manage any disease or condition, determine a stage of disease, or replace an individualized assessment, treatment plan, or care plan.
No professional relationship
Purchase, receipt, or use of these materials does not create a physician-patient, clinician-patient, therapist-patient, or other professional relationship with the author, LE VERE, INC., or any distributor of the materials. The author and LE VERE, INC. are not providing licensed healthcare services through these materials.
Individualized instructions control
Needs, diagnoses, medications, abilities, and risks vary from person to person. Do not use these materials to start, stop, change, or independently determine any medication, dose, timing, diet, food texture, swallowing precaution, mobility or transfer technique, supervision level, or other treatment or safety plan. Follow the individual's current care plan, medication instructions, and directions from appropriately licensed healthcare professionals. If these materials conflict with individualized professional instructions, the professional instructions control.
Symptoms and emergencies
These materials are not a diagnostic or triage tool and cannot determine whether a symptom is caused by dementia or another condition, whether a symptom is urgent, or whether care can safely wait. Seek prompt medical guidance for new, worsening, or concerning symptoms. For a medical emergency or an immediate threat to health or safety, call 911 or the applicable local emergency service. Do not delay professional care because of anything in these materials.
Research, stages, and suggested approaches
References to research, evidence, common patterns, stages, or suggested approaches are general educational summaries only. They do not guarantee effectiveness, describe every person with memory loss, or establish that a particular approach is appropriate for a specific individual. Dementia and memory-loss conditions vary substantially by person and diagnosis.
Facility or institutional use
In a facility, agency, or other professional-care setting, these materials are supplemental organizational aids only. They do not replace applicable law, facility policies and protocols, required assessments, scope-of-practice rules, staffing or supervision requirements, medication-administration requirements, professional judgment, or an individualized plan of care.
Stop if unsafe or inconsistent with the care plan
No particular result is promised or guaranteed. Stop using any sheet, instruction, or technique that appears unsafe, causes distress, conflicts with an individualized care plan, or is inconsistent with the instructions of a qualified healthcare professional.
Common questions
Questions FAQ
Short answers to what families, care partners and care teams most often ask. If your question is not here, write to hello@memorycaresheets.com.
What is CareSheetsCo?
CareSheetsCo makes printable one-page care sheets for families and care teams going through hard seasons of life. Each sheet does one job, uses one idea per line, and is written to be useful on a bad day, when reading, remembering and deciding are hardest. The first collection is Memory Care Sheets, for people living with memory loss and dementia and the people caring for them. A collection for grief is in development.
What is a care sheet?
A care sheet is a single printed page that holds something a person would otherwise have to keep in their head. It might be a step-by-step card for getting dressed, a list of who is helping with which hour of the day, or a record of what has already been decided. CareSheetsCo sheets are designed to be written on, taped to a wall, and read at a glance. They are organizing tools, not medical advice.
Who are CareSheetsCo sheets for?
CareSheetsCo sheets are for the person going through a hard season and for the people supporting them. In the Memory Care Sheets collection, that means people living with memory loss, family care partners, and the staff of care communities and home care agencies.
What care sheet collections are available?
Memory Care Sheets is available now, with sheets for every stage from early memory changes to late-stage dementia. A grief collection is in development and will follow the same design: one page, one job, sorted by what the person is facing now.
What are Memory Care Sheets?
Memory Care Sheets are printable one-page sheets that help a person living with dementia, and the people caring for them, get through daily life with less to hold in mind. The collection covers memory loss from early changes through late-stage dementia, with each sheet labeled by the stages where it helps most. Sheets include step cards for daily tasks, a daily "already done" card, a 24-hour care roster for sharing the load, decision records, and sheets for appointments, medicines, safety and change over time.
Are there printable dementia care sheets for each stage?
Yes. Every Memory Care Sheet is labeled by stage, so you can see at a glance what fits this week. The sheets are sold in three sets: the Person set, for the person living with memory loss to hold; the Caregiver set, for the person helping; and the Full set, which is both parts together.
How do I know which Memory Care Sheets I need?
Start with the free Sheet 0 check on the CareSheetsCo site. It asks what has happened in the last seven days, using plain tick boxes, and recommends the sheets that fit and the smallest set that holds them. Sheet 0 is free to everyone, and nothing you tick leaves your device.
What do the dementia stages on the sheets mean?
The stage labels follow a widely used scale that describes Alzheimer's-type memory loss in steps, from early changes (Stage 2 to 3) through late-stage dementia (Stage 6). They help sort the sheets so you see what fits this week rather than everything at once. Not every type of dementia follows the same order. Lewy body dementia and frontotemporal dementia, for example, can bring changes in a different sequence, so the Sheet 0 check works from what is actually happening, not from a diagnosis label. Only a clinician can stage a diagnosis.
Why are there two parts, one for the person and one for the care partner?
Memory Care Sheets come in two parts because the person living with memory loss and the person caring for them need different things on the page. Part One is for the person: large type, first person, one idea per line. Part Two is for the care partner: records, rosters, and plans that are written on and read back for patterns. Keeping them apart means the sheets on the person's wall speak to them with dignity, and the planning stays with the care partner.
Can someone with early memory loss use Memory Care Sheets on their own?
Yes. The Sheet 0 check has a path for people noticing memory changes in themselves, and Part One is written in the first person for the person to hold and use. Many early-stage sheets work best when the person fills them in while it is still easy, so the information is ready later.
Do Memory Care Sheets replace medical advice?
No. Memory Care Sheets are an organizing tool, not medical advice. The care plan comes from the clinician, and the sheets help a family hold and follow it.
How much do CareSheetsCo sheets cost?
CareSheetsCo sheets are a one-time purchase: the Person set is $15, the Caregiver set is $25, and the Full set is $35. The price is shown next to each set name on this site and again on Etsy before you pay. Sheet 0, the check that tells you which sheets you actually need, is free to everyone.
Is CareSheetsCo a subscription?
No. You pay once and the sheets are yours to print for as long as you need them. Nothing renews, and there is no account to cancel.
Do I need an account to buy or download?
No. Purchases happen on Etsy. Etsy emails your download link as soon as the payment clears, and the files also stay in your Etsy purchases. There is no CareSheetsCo account and no password to remember.
What if I lose my download?
Open your Etsy purchases and the files are still there. If you cannot find them, message the CareSheetsCo shop from the listing and the links will be sent again. Reprinting is part of what you paid for, for as long as you need it.
What happens when the person I care for moves to a later stage?
Run the check again and it names the sheets that fit now. Each set is a separate one-time purchase, so nothing carries over and nothing expires. The sheets you already bought keep working for as long as you print them.
How do I print Memory Care Sheets?
Memory Care Sheets print on any home printer on US Letter paper. Cream or pale yellow matte paper reduces glare and is easier for many people living with dementia to read, but plain white works too. Every stage color is also named in words, so the sheets work in black ink only. Daily cards can be printed several at a time, or laminated and used with a wipe-off pen.
Can I share the sheets with my family?
Yes. A CareSheetsCo personal purchase covers everyone helping with one person's care in that household: family, friends, and paid helpers who come to the home can all use and print the sheets for that person. Please do not post the files online, resell them, or share them outside that circle of care.
Does CareSheetsCo license sheets to care facilities and agencies?
Yes. CareSheetsCo offers a one-time site license for memory care communities, assisted living, and day programs, sized by the number of residents served, and an agency license for home care and hospice teams. A site license covers printing for every resident at that address. Facility licenses are invoiced, with a purchase order if you need one. The license is one-time to the set purchased: one license per set, at the resident count at time of purchase, not a forever-license to everything released later. Contact CareSheetsCo for a quote.
Can staff use Memory Care Sheets for training?
Memory Care Sheets work well alongside required dementia training, because a new aide learns the practice while using the step card. They support training and do not replace it or any state-required hours.
Does the CareSheetsCo quiz collect health information?
No. The Sheet 0 check runs entirely in your browser, and nothing you tick is sent to CareSheetsCo or anyone else. When you close the page, your answers are gone.
What information does CareSheetsCo keep?
CareSheetsCo keeps only what a purchase needs: your email, the set you bought, the date, and the amount. It is used to send your download links and nothing else. Etsy handles the payment itself, so we never see or store your card details.
An organizing tool, not medical advice. The care plan comes from the clinician.
